VT Army National Guard Personnel Record Request: For Soldiers and retirees discharged on or after 1 January 2004, please create a DS Logon and navigate to iPERMS to obtain your personnel record.

For personnel records prior to 1 January 2004 or records not shown in iPERMS, please complete a Standard Form 180. Please include as much information as possible, to include your contact information. If you are not the Service member, supporting documentation (Power of Attorney, Death Certificate of Service Member, Release signed by the Soldier if still living) is required. There are three options to send the request.

To request military records, please complete a Standard Form 180. At a minimum, please include as much information as possible, to include your contact information. VT Air National Guard Record Requests: For ANG members who were discharged, separated, or retired AFTER Oct. 1, 2004, please create an account and request your military record via www.ebenefits.va.gov or call the Air Reserve Personnel Center at 1-800-525-0102.  For those who were discharged, separated, or retired BEFORE Oct. 1, 2004, please request your military record by submitting a Standard Form 180.
