Military Records Request:
To request military records, please complete a Standard Form 180. At a minimum, please include Name, Social Security Number, Date of Birth and Place of Birth.
There are two options to send the request.
For ANG members who were discharged, separated, or retired AFTER Oct. 1, 2004, please create an account and request your military record via www.ebenefits.va.gov or call the Air Reserve Personnel Center at 1-800-525-0102. For those who were discharged, separated, or retired BEFORE Oct. 1, 2004, please request your military record by submitting a Standard Form 180.
Do not transmit any sensitive Personally Identifiable Information, such as your Social Security Number, via the contact form below. Any forms requiring sensitive PII should be faxed or mailed to the recipient.
